In today’s managed care environment, health insurers often seek to recoup payments made to providers on the alleged ground that these providers improperly or fraudulently billed for the services rendered.  The managed care companies often audit medical practices, and then contend that providers billed for higher-level services that are not supported by the medical record documentation.  They then use that on an extrapolated basis to obtain refunds of claims payments going back over a several year period.
